On a domain already on ACME, editing Additional Hostnames saves fine but nothing gets ordered. The new names only show up when the existing renewal comes due, about 60 days out on a 90-day cert with the default setting.
crates/jmap/src/registry/mapping/domain.rs only schedules AcmeRenewal when a domain switches to Automatic from something else. It should schedule one when the names change too, and probably the provider. For now you can set the domain to manual, save, then set it back to ACME and save, though that probably leaves the old pending task next to the new one.
